We are a part of your Patient-Centered Medical Home Neighborhood! A Patient-Centered Medical Home (PCMH) is a system of care in which a team of health professionals work together to provide all of your health care needs. You, the patient, are the most important part of a patient-centered medical home. When you take an active role in your health and work closely with us, you can be sure that you're getting the care you need.


Patient - Physician Agreement

Providing the best possible care to every patient continues to be our primary goal. One way we can achieve this objective is working together to make your health and well-being our number one mutual priority. We have outlined the following patient and physician responsibilities to help meet this goal:


Patient Responsibilities:

Ask questions, share your feelings and be part of your care.

Please be sure that you are current and accurate with your medical history, symptoms and other important information about your health.

Tell us about any changes in your health.

Make healthy decisions about your daily habits and lifestyle.

Keep your appointments and to know your insurance and what it covers. We expect you to pay your share of the visit when seen in the office.

End every office visit with a clear understanding of our mutual expectations, treatment goals and future follow-up visit plans.


Physician Responsibilities:

Explain diseases, treatments and results in an easy-to-understand way.

Provide safe quality care to you when needed, with respect to you and your privacy.

To care for you to the best of our abilities based on our understanding of current medical methods available.

Return telephone enquires within 24 hours, if possible.

Complete medical records requests within 7-10 business days.

Discuss the most appropriate tests and procedures you need to meet your goals and coordinate your care among other health care professionals.

Mon, Wed, Fri: 10:00-1:00 & 3:00-6:00 | Tues & Thur: 2:00-7:00 | Sat: 10:00-12:00


After Hours Care:

If you have health care needs after hours, please contact your primary care physician to guide your care or for advice pertaining to your health situation. If you feel your condition is an urgent one, you may find one of the following urgent care facilities to be appropriate and convenient:

If you feel your condition is a medical emergency, please call 9-1-1 or proceed to your nearest emergency room.

2-1-1 is available throughout most of Michigan. Dial 2-1-1 from any phone and you will be connected with a referral hotline that can connect you with non-profit agencies in the area that can help with human, health and social needs (i.e., utilities, housing, insurance, food, etc.).
